Serves as a pharmaceutical caregiver by efficiently designing, recommending, activating, monitoring, and evaluating patient-specific pharmacotherapy to ensure optimal outcomes. Ensures implementation of the pharmaceutical care plan by assuring the accurate and timely distribution of medications to patients. Serves as a clinical resource for other pharmacy personnel, medical and nursing staff, as well as other healthcare professionals.
Positions available in the following departments: Critical Care, Oncology, Pediatrics, Infectious Diseases, Transplant, Cardiology, Medical/Endocrinology.
Requirements:
- BScPharm, Master's Degree or Doctor of Pharmacy (with residency) required
- Current licensure as a Clinical Pharmacist in Canada or the United States, and
- For those with a Bachelor's degree in Pharmacy: Six (6) years recent full-time hospital pharmacy practice experience, including three (3) years of current full-time experience in clinical capacity (in one of the above noted specialty areas) in a large North American acute-care hospital.
- For those with a Master's in Pharmacy or PharmD: Must have completed a residency and have a minimum of two (2) years of current full-time hospital pharmacy practice experience in clinical capacity (in one of the above noted specialty areas) in a large North American acute-care hospital.
