Become part of an innovative way of practicing family medicine in the GTA!

This new clinic is based on a primary, family-centred health and wellness care clinic model.

It will focus on providing quality care, preventative care, patient teaching, and patient advocacy.

Its team of family physicians, nurse practitioners, and case managers (patient coordinators) will be supported by innovative strategies and software, which will help ensure that patients are following their treatment programs and receiving prompt referrals and access to necessary community services.
